This page contains detailed instructions on how to fill out the forms you need to submit. You may download a form by clicking on its name.

IMS Form 01: Conformance Registration Agreement ( Word ) ( PDF )

This form is always required. Note that this form must be hand-signed and submitted to the Evaluation Organization via fax or postal mail only.

1. Fill in your organization in the Vendor field.
2. Fill in your Product Name and Version/Release Number.
3. Have an authorized person sign the bottom of the form in the For Vendor section.

IMS Form 02: IMS Product Conformance Claim Form ( Word ) ( PDF )

This form is always required.

1. Complete all fields in the Basic Information section.
• Make sure to choose the Product Type that best represents your product.
• Note that only the contact listed in the Submitted By field will be authorized to correspond with the Wisconsin Testing Organization regarding the evaluation.
• Ensure that you check ALL classes that your product belongs to in The Product field.
2. In the Supported Specifications section, place a check mark or X by ALL specifications in the table that are being claimed.
• Please verify the version number corresponding to each specification you mark to make sure this is the version you want to claim your product is conformant to.
• You may write-in any older version of the listed specifications if the version you want to claim is not listed.
• Optionally, enter any special comments or considerations in the General Comments field. You may use additional pages as necessary.

IMS Form 03: IMS Content Collection Conformance Claim Form ( Word ) ( PDF )

This form is only required if you plan to claim conformance for a Content Collection.

1. Fill in the Product Name and Product Version from IMS Form 02.
2. Describe the conformance claim as per the given instructions.
3. Enter evidence pertaining specifically to the content collection as a whole on Page 2 in the "Evidence" section.

IMS Form 04: IMS Evidence Registration Form ( Word ) ( PDF )

Fill out one IMS Form 04 for each specification you claimed on IMS Form 02.

1. Fill in the Product Name and Product Version from IMS Form 02.
2. Fill in the Specification Name and Specification Version.
3. Fill out section A if you indicated on IMS Form 02 that your product is a Data Instance.
4. Fill out section B if you indicated on IMS Form 02 that your product is an application that imports IMS-defined data or content.
5. Fill out section C if you indicated on IMS Form 02 that your product is an application that exports IMS-defined data or content.
6. Fill out section D if you indicated on IMS Form 02 that your product supports IMS Web Service interfaces.
